Background Check Authorization Agreement

By continuing the signup process and registering as a Helper on the Favor On-Demand platform, you authorize Favor On-Demand LLC (“Favor On-Demand,” “we,” “our,” or “us”) and its third-party background check provider, Checkr, to conduct a background check for the purpose of verifying your eligibility to provide services through the platform.

You acknowledge and agree that the background check may include, but is not limited to:

• Identity verification
• Criminal history records
• Public records searches
• Address history verification
• Other legally permissible screening checks

These checks may be conducted through Checkr, a third-party consumer reporting agency.

By agreeing to this Background Check Authorization Agreement, you provide your consent for Favor On-Demand and Checkr to obtain consumer reports and background reports about you for the purpose of determining your eligibility to participate as a Helper on the platform.

You authorize Favor On-Demand and Checkr to verify the information you provide and to obtain background information from public and private sources as permitted by law.

You acknowledge that Favor On-Demand may request additional or periodic background checks during the time you use the platform as a Helper.

Your continued use of the platform constitutes ongoing authorization for such checks.

Completion of a background check does not create an employment relationship between you and Favor On-Demand.

Helpers are independent contractors and are not employees, agents, or representatives of Favor On-Demand.

Favor On-Demand does not make hiring decisions on behalf of Helpers or Requesters.

Background checks are conducted by Checkr, and Favor On-Demand does not guarantee the accuracy, completeness, or reliability of the information contained in any background report.

Favor On-Demand is not responsible for errors or inaccuracies contained in background check reports provided by third parties.

If you believe that information contained in your background report is inaccurate or incomplete, you have the right to dispute the information directly with Checkr.

Instructions for disputing a report will be provided by Checkr during the background check process.

Background checks are conducted in accordance with applicable laws, including the Fair Credit Reporting Act (FCRA) and other relevant federal and state regulations.
